The mission of this practice is:
-
to provide quality counseling from a biblical world view.
-
To help you create life-giving relationships with oneself, others, and God.
Your first appointment
Your first appointment will include an intake session. In the session we will talk about what brings you to counseling. I will also collect some important information about you including things such as your family history, medical history, and any past counseling you’ve had. We will talk about confidentiality, and sign forms related to this. You can view these forms below. Before the session ends we will set some basic goals for treatment. You can expect this session, and the following sessions, to each last about an hour.
What to bring
Medication information
Print and fill out the above three forms. This will save time we can use more productively in our first session.
